  • Certified Copy of Death Certificate
  • Copy of Nomination Form
  • Police Report in the event that the death was accidental
  • Affidavit in the event of difference in surnames

BOIKANYO FUNERAL PLAN:

  • Starting from as little as P90 per month.
  • Comprehensive Coverage: Main Member & Spouse: Cover up to P100,000 per policy, with up to 4 policies allowed.
